The kidney handles acid by reabsorbing bicarbonate and excreting hydrogen

Kidneys are the body’s acid janitors, scrubbing the sink and flushing the drain.
The idea behind the cue
The kidneys regulate acid-base balance by reclaiming bicarbonate and secreting hydrogen ions. In renal failure, metabolic acidosis can develop because acid excretion falls and bicarbonate conservation is impaired. This is why renal disease often affects breathing patterns, labs, and overall metabolic stability.

Short & simple
Kidneys are the body’s acid janitors, scrubbing the sink and flushing the drain.
Acronym
BHB — Bouncy Hedgehogs Burp.
Story
A cleanup crew first grabs reusable soap from the sink, then tosses acid sludge down the drain. When the crew quits, the room smells sour and everything corrodes. The kidney normally keeps blood pH balanced by reabsorbing bicarbonate and secreting hydrogen.
Connect the pieces
- B (Bouncy) → Bicarbonate reclamation: Kidneys reabsorb bicarbonate to help defend the body against acid.
- H (Hedgehogs) → Hydrogen secretion: Kidneys excrete hydrogen ions to remove acid from the body.
- B (Burp) → Buffer failure: When renal failure reduces acid excretion and bicarbonate conservation, metabolic acidosis can develop.
- Bouncy · Bicarbonate reclamation: Kidneys reabsorb bicarbonate to help defend the body against acid.
- Hedgehogs · Hydrogen secretion: Kidneys excrete hydrogen ions to remove acid from the body.
- Burp · Buffer failure: When renal failure reduces acid excretion and bicarbonate conservation, metabolic acidosis can develop.
Know the limits
This card addresses renal acid-base control, not respiratory compensation mechanisms.

Which renal function most directly helps prevent metabolic acidosis?
A. Reabsorbing bicarbonate
B. Producing digestive enzymes
C. Increasing insulin release
D. Storing glycogen for fasting
Reveal explanation
Reabsorbing bicarbonate
Bicarbonate reabsorption helps buffer blood acid; loss of this renal function contributes to metabolic acidosis.
