Swollen ankles, bulging neck veins
The idea behind the cue
Right-sided heart failure causes systemic venous congestion. Typical findings include peripheral edema, jugular venous distention, hepatomegaly, ascites, and weight gain from fluid retention. It often develops after left-sided failure or from lung disease that increases right ventricular strain.

Short & simple
Swollen ankles, bulging neck veins
Acronym
RJHA — Rubbery Jellyfish Hide Accordions.
Story
A bridge closes and traffic piles up behind it. The backup is behind the blockage, not beyond it. In right-sided heart failure, blood backs up into the systemic veins, causing swollen legs and distended neck veins.
Connect the pieces
- R (Rubbery) → Right-sided failure: The right heart fails to move blood forward, causing systemic venous congestion.
- J (Jellyfish) → JVD: Jugular veins distend because venous blood is backing up.
- H (Hide) → Hepatomegaly: The liver enlarges from venous congestion.
- A (Accordions) → Ascites: Fluid can collect in the abdomen, often along with peripheral edema and weight gain.
- Rubbery · Right-sided failure: The right heart fails to move blood forward, causing systemic venous congestion.
- Jellyfish · JVD: Jugular veins distend because venous blood is backing up.
- Hide · Hepatomegaly: The liver enlarges from venous congestion.
- Accordions · Ascites: Fluid can collect in the abdomen, often along with peripheral edema and weight gain.
Know the limits
Edema has many causes; this pattern is most suggestive when paired with JVD and other congestion signs.

Which finding is most characteristic of right-sided heart failure?
A. Crackles with pink frothy sputum
B. Stabbing chest pain that worsens with inspiration
C. Absent radial pulse in one arm only
D. Peripheral edema with jugular venous distention
Reveal explanation
Peripheral edema with jugular venous distention
Right-sided failure causes venous backup into the body, leading to edema and JVD. The other choices suggest pulmonary edema, pleuritic pain, or arterial obstruction.
