Short & simple
Red alarm: give the benzo first.
Acronym
BAM: Benzodiazepine, Airway support, Monitoring—three teammates for prolonged convulsions.
Story
A siren blares; a tiny wizard throws a benzodiazepine cape at the seizure monster while nurses snap on monitors and check the airway.
Connect the pieces
- Decode the mnemonic: BAM: Benzodiazepine, Airway support, Monitoring—three teammates for prolonged convulsions.
Know the limits
Emergency stabilization and monitoring accompany treatment; maintenance epilepsy therapy is different.
American Epilepsy Society · Convulsive status epilepticus guideline · 2016 ↗Drug guide: Seizures
Initial and second-line emergency treatment of convulsive status epilepticus; not routine maintenance epilepsy treatment.
- midazolam, lorazepam, diazepam: Initial benzodiazepine options
- fosphenytoin, levetiracetam, valproic acid: Options when convulsions persist after adequate initial therapy
Use IM midazolam, IV lorazepam or IV diazepam in the initial treatment phase while supporting airway, breathing and circulation. Persistent convulsions require prompt escalation.
IV phenobarbital is a second-phase alternative when the other recommended second-phase agents are unavailable.
- Route, age and weight determine dosing; a benzodiazepine name alone is not a complete order.
- Monitor ventilation and hemodynamics; do not confuse rescue treatment with long-term seizure prevention.

A. Benzodiazepines
B. Sodium-channel blockers
C. Carbonic anhydrase inhibitors
D. Alpha-2-delta ligands
Reveal explanation
