Too much dopamine in the gut makes nausea the gremlin
The idea behind the cue
Carbidopa inhibits aromatic L-amino acid decarboxylase outside the CNS, reducing peripheral conversion of levodopa to dopamine. This helps increase central delivery and lessen peripheral adverse effects such as nausea. It is not used alone for Parkinson disease because it does not meaningfully treat symptoms by itself.

Short & simple
Too much dopamine in the gut makes nausea the gremlin
Acronym
PDB — Polka Donuts Bounce.
Story
A suitcase of levodopa gets mobbed by greedy enzymes in the airport lobby. Carbidopa stands at the door and stops most of the sabotage outside the brain, so more levodopa reaches the neurologic destination.
Connect the pieces
- P (Polka) → Peripheral: Carbidopa acts outside the CNS, where it blocks peripheral levodopa-to-dopamine conversion.
- D (Donuts) → Decarboxylase: It inhibits aromatic L-amino acid decarboxylase, limiting peripheral breakdown of levodopa.
- B (Bounce) → Brain: This helps more levodopa reach the brain and can lessen peripheral adverse effects like nausea; it is not a stand-alone Parkinson treatment.
- Polka · Peripheral: Carbidopa acts outside the CNS, where it blocks peripheral levodopa-to-dopamine conversion.
- Donuts · Decarboxylase: It inhibits aromatic L-amino acid decarboxylase, limiting peripheral breakdown of levodopa.
- Bounce · Brain: This helps more levodopa reach the brain and can lessen peripheral adverse effects like nausea; it is not a stand-alone Parkinson treatment.
Know the limits
Acts only peripherally; it is not a stand-alone Parkinson treatment.

A patient asks why carbidopa is included with levodopa. Which explanation best describes the purpose of the combination?
A. To block dopamine receptors in the striatum.
B. To treat acute psychosis from Parkinson medications.
C. To directly replace acetylcholine in the basal ganglia.
D. To reduce peripheral breakdown of levodopa and increase delivery to the brain.
Reveal explanation
To reduce peripheral breakdown of levodopa and increase delivery to the brain.
Carbidopa inhibits peripheral decarboxylation of levodopa, improving central availability and reducing peripheral side effects.
