Gut and liver customs stamp the pill before bloodstream entry
The idea behind the cue
First-pass metabolism is presystemic metabolism in the gut wall and especially the liver after oral absorption but before systemic circulation. It lowers the fraction of an oral dose that reaches the body unchanged and therefore reduces bioavailability.

Short & simple
Gut and liver customs stamp the pill before bloodstream entry
Acronym
FPL — Fluffy Pancakes Lounge.
Story
A pill boards the gut subway and rides straight to liver customs through the portal vein. Security confiscates part of the cargo before the train reaches the city, so less unchanged drug enters systemic circulation.
Connect the pieces
- F (Fluffy) → First-pass: Oral drug absorbed from the gut can be metabolized before systemic circulation, so less unchanged drug gets through.
- P (Pancakes) → Portal route: Oral and many enteral drugs drain into portal blood on the way to the liver.
- L (Lounge) → Liver: The liver is the major presystemic site; IV dosing bypasses this first-pass step.
- Fluffy · First-pass: Oral drug absorbed from the gut can be metabolized before systemic circulation, so less unchanged drug gets through.
- Pancakes · Portal route: Oral and many enteral drugs drain into portal blood on the way to the liver.
- Lounge · Liver: The liver is the major presystemic site; IV dosing bypasses this first-pass step.
Know the limits
This mainly affects oral and some enteral drugs that drain to the portal circulation; IV dosing bypasses first-pass metabolism.

After an oral dose, much of the drug is metabolized before unchanged drug enters systemic circulation. Which process explains the reduced systemic exposure?
A. First-pass metabolism
B. Zero-order elimination
C. Protein binding
D. Therapeutic index
Reveal explanation
First-pass metabolism
First-pass metabolism occurs before systemic circulation and reduces the amount of unchanged oral drug that reaches blood.
